AGREEMENT OF RELEASE AND WAIVER OF LIABILITY
By checking the box below, I agree that I am participating in classes or workshops offered by Mysore Yoga Atlanta, LLC (“Mysore Yoga Atlanta”). I recognize that yoga may be physically strenuous and I am voluntarily participating with full knowledge that there is risk of physical injury.
I understand that is my responsibility to consult a physician prior to and regarding my participation in yoga classes or workshops. I represent and warrant that I am physically fit and I have no medical condition which would prevent my participation in the classes or workshops.
In consideration of being permitted to participate in Mysore Yoga Atlanta classes and workshops, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result such participation.
In further consideration of being permitted to participate, I knowingly, voluntarily and expressly waive any claim I may have against Mysore Yoga Atlanta and its instructors, for any injury or damages I may sustain as a result of participating in the classes or workshops and I agree that neither I, my heirs or legal representatives will sue or make other claims of any kind whatsoever against Mysore Yoga Atlanta or its instructors for any injury, damages or wrongful death, whether caused by negligence or otherwise. Mysore Yoga Atlanta is in no way responsible for the safekeeping of my personal belongings while I attend class.
I understand that during class instructors may physically adjust me, and I agree that the above release and waiver of liability also extends to any claim whatsoever on account of adjustment rendered during my participation in yoga classes or workshops.
I give Mysore Yoga Atlanta permission to use and reproduce any photographs, personal narrative, interviews or audio and video recording of my participation in classes or workshops on its website and for any other purposes without compensation.
I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions above and recognize that my checking the box below serves as a complete and unconditional release of all liability to the greatest extent permitted by law in the State of Georgia.
